A toolkit on how to implement social prescribing

Social prescribing is a means of connecting patients to a range of non-clinical services in the community to improve their health and wellbeing. This toolkit was created to help introduce social prescribing at the community level. It illustrates the steps needed to introduce a social prescribing programme and includes example materials that can be adapted to the local environment. It can be used by, among others, implementing organisations, health and long-term care facilities, mental health and healthcare professionals. Policy makers and health and social authorities may also find this resource useful for the extension of community interventions.
